When you attend a seminar, we will provide you additional information about the program to help you decide if you are ready to move ahead. Some people choose not to go forward, some choose to think about it more, and some people are eager to proceed. Whatever your choice, it’s ok. If you are ready to move forward with the process, you will be given several items of paperwork to complete before your first appointment. The Bariatric Surgery Patient History asks for information that we need to substantiate a history of being severely overweight, attempts to lose weight by more “traditional means” and the result of those efforts. There is also detailed health history including current health problems, previous surgeries, and medications. We create your personalized care plan based on the information that you provide, so we appreciate accurate and detailed information. Please review lables on your medication bottles, or bring the containers with you to your appointment. Please complete all of the paperwork before your sheduled appointment.
